Scenario
Mr Marshall, 68 years old male resident at fairfax virginia was brought at the hospital by his daughter. He presents with fever, headache, chills and mild cough. He was well until 3 days earlier, when he suffered the onset of a nasal stuffiness, mild sore throat, a cough productive of small amounts of clear sputum and diarrhea. He came in today because of an increase in temperature to 102 degree and spams of coughing that produce purulent secretions. He passed swallowing test. Chest radiography documents bilateral lower lobe infiltrates that are more pronounced on the right side. There are no pleural effusions.
Medical history: He has a history of chronic kidney disease, hypertension. He is allergic to codeine.
Vitals: His vitals were as follow : BP:118/68 , Temp :102 , HR: 18 breaths/min , Pulse ratev: 110 beats/ min0xygen sat : 93% room air , There is mild erythema of the mucosa of the nose. Ins piratory “rales” are heard at the right lungs.
ABGs = PH : 7. 20 , PCO2 : 52, BICARB : 35
LABS= HEMOGLOBIN : 12.4 , HEMATOCRIT: 37.3, PLATELET: 97, RBC: 39, BUN: 24.0, CREATININE: 1.3, CO2: 18, ALBUMIN: 2.5, GFR: 53.1
Patient is diagnosed with PNEUMONIA .
Plan: Incentive spirometer, Monitor vitals, admitted, thrombocypotenia
