ale, age 58. History of silicosis after working in an asbestos plant for 40 years. Brought to emergency department after his wife could not arouse him this morning. He is lethargic, yet restless. He is sitting up in bed leaning his arms and head on the overbed table. He repeatedly removes the oxygen cannula and talks about calling his wife to bring him his baseball cards, although she is present in the room with him. His lung sounds are diminished in all fields, and the percussion note is hyper-resonant. His pulse is 114 and irregular, and respirations are shallow at 28. His temperature is 98.2 F and his BP is 96/64. He has 2+ pitting edema below the knees. His face is flushed, but he is exhibiting circumoral cyanosis. Laboratory values: Hct: 45% Hgb: 15.2 g/dl K+: 5.4 mEq/L What acid base imbalance is he experiencing?
