GC is a 33 year old white female with a history of anxiety disorder treated by her PMHNP with alprazolam 0.25 mg three times daily and sertraline 50 mg daily. Her dose of alprazolam has been unchanged for several years, this has never caused her to have somnolence or inattentiveness. Today GC presents to clinic with new onset of somnolence and is hardly able to keep her eyes open. She relates that she has recently had a vaginal yeast infection. The over the counter vaginal yeast cream that she normally uses did not seem to be working and so her sister gave her one pill to take orally. The sister had received the prescription from her provider who told her that only one dose was usually needed, but gave her a refill if she needed a second dose. The sister didn’t use the refill; she gave it to GC. GC took the tablet yesterday evening and today was so groggy that she had to call in sick. She is worried and unable to decide what to do; she wants to know what happened to make her so sleepy all of the sudden?
