Dominic, 7-year old male, was brought into his pediatrician’s office by his mother after falling off the monkey bars at the playground six weeks ago. His left forearm is in a cast, healing from a Greenstick fracture of the ulna. His mother tells Dr. Katz that Dominic has not been eating, has been unable to concentrate, and he appears to always be cold, no matter the temperature. She is worried that Dominic is not getting proper nutrition. After a complete physical exam and speaking with Dominic privately, Dr. Katz sends Dominic down the hall for an x-ray to check the healing of his fracture. The results show delayed healing and Dr. Katz determines this is due to Dominic’s malnutrition. The principal (first-listed) diagnosis code to report is: S52.212G Greenstick fracture of shaft of left ulna, subsequent encounter for fracture with delayed healing S52.212P Greenstick fracture of shaft of left ulna, subsequent encounter for fracture with malunion E44.0 Moderate protein-calorie malnutrition E46 Unspecified protein-calorie malnutrition (Malnutrition NOS) S52.212G E46 E44.0 S52.212P
