response to Pain is “an unpleasant sensory and emotional experience associated with actual or potential tissue damage” (Lewis’s, 2022, p. 120). Pain is a subjective, it cannot be directly observed by another person. Pain is base off of what the person in pain is experiencing and how they are feeling at the moment when they are experiencing the pain. At times when discussing pain, it can make it hard for providers to determent was level pain the person it truly in, most time the provider or medical staff will ask the patient what is their pain level on a scale from 0-10 and whatever the patient reports you will document and treat patient base that. Other time as nurses we might encounter patient who mentally challenged or a disability, which can include someone who had a stroke and cannot speak, in a coma, has a brain injury, or dementia. Most time with these patient they will not be able to tell you what pain they are encountering, which can make your plan of care for the patient a little harder to manage. But you can always base their pain level off if their facial expression, current diagnosis,
