Nurses’ Notes 0830: Client telephoned clinic with reports of involuntary movements. Reports noticing movement of face and lips over the past week, accompanied this morning by tongue thrusts. Client instructed to come to clinic for evaluation. 1000: Client reports to clinic. Oriented to person, place, and time. Walks with altered, rocking gait. Purposeless movement of hands and fingers, facial grimacing, and tongue thrusting noted. Client denies any substance use. Reports taking prescribed medications as ordered.
