Your ICU patient is scheduled to undergo abdominal surgery. Since patient is allergic to Penicillin, you ordered a loading dose of Vancomycin 1.5 gram to be administered before surgery. The pharmacy sent the dose 30 minutes before surgery and the nurse programmed Vancomycin to be infused over 20 minutes so that the entire loading dose maybe loaded in before surgery. 10 minutes into the infusion, the patient started itching, complained of feeling nauseated, and developed red rash over his upper torso and face. What happened and how could this have been prevented?
