History of Present Illness Patient is currently encephalopathic and unable to provide any history. History obtained from patient’s chart and from ED personnel caring for her. Unable to reach patient’s sister via phone. Ms. Brown is a 64-year-old woman with medical history pertinent for hypertension and diabetes who was reportedly in her usual state of health until about a week ago when she developed fever and dyspnea. She was evaluated in urgent care center but no specific diagnosis was made. Her sister checked on her today and found her quite altered prompting her transportation to the ED.
