Child is being seen today for recent reports of lethargy. Child was diagnosed with Down syndrome at birth. They have been highly functional with good levels of energy until recently. Parent states that within the last month, they have noticed a decrease in their child’s energy level. Child usually participates in sports and is out riding their bicycle, very active. They have also noticed some weight gain, 10 lb. (4.5 kg), in the last month. Physical assessment was completed and charted. Vital signs: temperature, 96.6° (35.9°C); pulse, 56 beats/min; respiratory rate, 18 breaths/min; blood pressure, 100/60 mm Hg. Oxygen saturation, 98% on room air.
