A 45 year old female patient presents to the ER following a motor vehicle accident twelve hours ago. They have low back pain that radiates down their left leg. She ranks the pain at 10 out of 10 on a 1 to 10 scale, describes it as “sharp” and does get some relief with rest. The patient admits to both bladder and bowel incontinence which is new. As you examine the patient you notice that they have decreased sensation of both inner thighs in a saddle riding pattern, and this is known as “saddle paresthesia”. An MRI is obtained emergently: Thinking about your studies this week, could this possibly be related to the cauda equina? Why? What is the treatment?
