Ms Goya is a healthy 19-year-old female university student who regularly attends the gym.  Ms Goya has noted increased shortness of breath and decreased energy during the past four months, exacerbated during her menses. Her GP noted tachycardia on an electrocardiogram trace, but physical and laboratory examinations two months ago were otherwise fine. The GP suggested that her symptoms were caused by either premenstrual syndrome or stress from her study but assured her that there was nothing physically wrong. 

  

Current status 

Today, at the gym, Ms Goya light-headed, suffering a temporary loss of consciousness, and was taken by ambulance to the local hospital.  

On admission to the emergency department the following were noted 

Temperature 37°C 

Heart rate140 BPM and regular 

Respiration 28 breaths/min 

Blood pressure 120/80 mmHg. 

Slender female, 170cm, 55kg. Resting quietly, slightly nervous, no acute distress. Skin is warm; conjunctiva is pale ; tongue is smooth, mild cracking of the lips.
