Mrs. Sanchez is a 57-year-old Latino woman admitted with repeated attacks of severe pain in the left upper quadrant that radiates to her back. She reports having vomited repeatedly over the past 24 hours. When the pain is severe, she reports feeling flushed, faint, and short of breath. She has no other related symptoms. Mrs. Sanchez reports having had a gallbladder attack several months ago that resolved without surgery. She is a high school teacher who lives with her husband.
Vital signs: blood pressure 144/84 mm Hg, pulse 92 bpm, Ht 5’2″, Wt 150 lbs, respiration 24 breaths per minute, temperature 100.2°F, measured orally. The patient appears to be in acute distress but is alert and oriented. Abdomen is tender and distended, with hypoactive bowel sounds in all four quadrants.
Patient was admitted to Surgical Unit and is being evaluated and prepared for surgical intervention. The following orders were given:
1. Monitor vital signs every 2 hours
2. Insert Nasogastric tube and connect to low suction
3. Intake and Output every 4 hours
4. CBC, BMP STAT
5. Start IVF D5 LR 1 L at 125 mL/hr continuous
6. Norco 10/325 1-2 tabs every 4-6 hours PRN for pain
7. NPO
