Michael Knight is a 48 year old white male with a past medical history for chronic kidney disease, hypertension, uncontrolled, diabetes mellitus, COPD and chronic pain, who presents in office today complaining about shortness of breath that he states has been present off and on for a few days. He states that he uses his inhaler occasionally. He denies cough, chest, fever, fatigue, change in appetite. He lives with his family, works construction periodically when he is able to find work. He smokes cigarettes and occasionally, marijuana. He denies illicit drug and alcohol abuse.
While reviewing his chart you note that he does not consistently visit the provider as scheduled and attempt to talk with him crosses him arm and states “it does not matter, no one ever listens to me anyway.”
