Your coding staff has been reporting an increase in the number of discharge dispositions that are incorrectly entered in the patients’ EHR by case managers and then have to make the change, which is causing a decrease in productivity and a corresponding increase in the discharged not final billed total (DNFB). The CFO of the hospital is very concerned with the negative impact this may have on the hospital and has asked you to have staff complete an audit by collecting data over the next two weeks.
1. Using CMS information, determine the correct discharge status for the seven error types identified based on the information below (see page 327 in your textbook and reference the MLN Matters article listed under resources for a complete list of Discharge Status Codes).
2. interpret the information and offer recommendations for error reduction to the Director of Case Management.
Coding staff identified the following on review of 198 discharged records in the two week period.
• Seaside Community Hospital – critical access hospital
• Seaside Children’s Hospital – children’s hospital
• Ocean View Hospital – long-term care hospital
• South Bay VA Hospital – veteran’s affairs hospital
• Wave Vista – inpatient psychiatric hospital
Errors found were:
• 12 transfers to Seaside Community Hospital as “04.”
• 5 transfers to Seaside Children’s Hospital listed as “02.”
• 7 patients listed as expired “41.”
• 3 transfers to Seaside Community Hospital swing bed listed as “50.”
• 2 patients discharged to jail listed as “10.”
• 4 transfers to Ocean View Hospital listed as “04.”
• 1 transfer to South Bay VA Hospital inpatient psych unit listed as “65.”
