Mr. Jones is an active, 80 yr old elderly man in a nursing home who has no close relatives except a young nephew. During a walk one day, he receives a puncture wound to the leg that becomes infected and gangrenous. As a result of his illness, Mr. Jones has a high fever and becomes unresponsive to questions. The physician decides that the only way to save his life is to remove his leg. The nephew tells the physician that he does not think his uncle would want to live if his leg was amputated. The patient has left no advance directives in regards to what he would wish in such a case.
