Fritz Mathew’s, is age 72, has had uncontrolled hypertension for the past two years. His healthcare provider has tried different anti hypertensives to control his disease.Mr Mathew’s has been admitted to the hospital four times in the last 6months. This is his fourth admission with acute heart failure. The physician explain to Mathew that hypertension that is uncontrolled can cause an excessive overload to the heart, thus decreasing the hearts ability to pump efficiently. Mr Mathew’s has recently lost his wife, and is leaving alone. He states that he forgets to take his medication at the prescribed times. He also eats a lot of frozen dinners and does not have the energy to cook his meals. Mr. Mathew’s has gained 10 pounds since his last admission. He has had difficulty sleeping at night and uses two pillows due to shortness of breath, he also reports that he frequently gets up at night to urinate.
