workshop example
Susan is a 50-year-old lady who lives at home. She has always been overweight, but now that she is in menopause, her weight has increased to 97kg, with her height being 163cm. She was diagnosed with Diabetes Mellitus about 2 years ago. Initially Susan did well with just one medication and diet and exercise. However, at her last two visits her Hb A1C was greater than 8% with fasting blood sugars of 15mmols. Therefore, Sitagliptin was added.
Her Medications:
Metformin 500 mg tablets PO BID
Sitagliptin 50mgs PO BID
1) Does Susan have Type 1 or Type 2 Diabetes mellitus? Provide a rationale for this decision. Compare and contrast Type 1 and Type 2 Diabetes mellitus.
2) Describe the pathophysiology of micro-vascular and macrovascular changes that are seen with Diabetic Mellitus. What symptoms would she have as these develop?
3) Describe the pathophysiology of diabetic neuropathy. What symptoms would she have as these develop? (5marks) 4) What is the rational of measuring HbA1c, Ketones, Glucose challenge test and fasting blood glucose?
5) Why does a diabetes patient with stress or infection always present with hyperglycemia? Why do people with T2 DM often present with recent history of rapid weight loss?
