Greg suffered a back injury at work two years ago which required laminectomy surgery for a herniated disc. This has now left him with persistent and ongoing lower back pain and sciatica down his right leg.
Greg’s Past Medical History:

             – Osteoarthritis

             – Depression

             – Body Mass Index (BMI) = 29 (overweight)

Greg is independent with usual Activities of Daily Living (ADL’s).
Greg has reduced mobility which restricts some of his normal physical activities. He can no longer play football, but enjoys some outdoor activities such as gentle walks and swimming.
The pain also restricts some of his usual work activities – Greg cannot stand for long periods of time and is unable to lift objects over 3kg in weight.
Greg reports he rarely attends social activities, has poor appetite, experiences difficulty sleeping and lacks energy/motivation at times.
