Case study is below:
Miley Stewart is an independent 91 year old woman who has lived in her house for the past 40 years. Her husband passed away 15 years ago and since then Miley has been independent. She stopped driving 1 year ago and relies on her family and friends to take her to appointments, for groceries etc. She is a very social and active lady who regularly goes to her church and who has lots of visitors.
Lately Miley’s family has noticed that she is becoming more forgetful and does not always remember to take her medications and can’t always recall if she had eaten meals etc. When questioned Miley is refusing to consider any retirement or assisted living accommodations, she plans to stay in her house ‘as long as she is mobile’. Her family is understandably concerned and is looking for solutions to help ensure their mother’s safety and independence. Miley acknowledges that sometimes she forgets things and is interested in working with you if “it will keep my family off my back” and supports her staying in her house (although she doesn’t really see the problem).
The health teaching plan includes Assessment, Objectives, Planning, Implementation and Evaluation.
