With the focus of health care moving towards meaningful use and value-based care models, a healthy community improves patient outcomes by keeping patients healthier in their own community and preventing hospitalizations (Centers for Disease Control and Prevention 2023).

The benefit of using a specific model or framework while completing a community health needs assessment allows the focus to follow an organizational methodology.  The framework allows the data to be presented in an organized fashion, while prioritizing issues, and allows for planning and evaluating outcomes (Centers for Disease Control and Prevention 2023).  Some action items in the plan will need completed before others can be started, however, some steps may be done simultaneously, this allows some control over the pace and timing of the community assessment.  The type of assessment being done can dictate which framework is selected.  An option to blend some frameworks together to create a hybrid framework for your community assessment is also available and sometimes is needed. (Centers for Disease Control and Prevention 2023).

Community health assessments should be a collaboration of local city agencies state agencies, stakeholders within the community and hospitals that provide services to those areas (Heath, 2018).  By working together for the community, the agencies and hospitals can identify areas of need, present improvement plans and educate the community on the needs identified from the assessment.  They can also collaborate on the evaluation and outcomes of the assessment.  Community health needs assessments are required by all tax-exempt hospitals, and they must submit their assessment to the IRS every 3 years as part of the Affordable Care Act (Heath, 2018). However, some data may take less time or more time to get an accurate picture of the community’s health needs.  There is no set standard questions or timelines to complete a community health assessment (Health, 2018). 

The role of the public health nurse is to advocate and educate within the community.  Their ability to assess, plan, gather data, and implement new community health needs allows them to be a valuable contributor to the assessment process.
