He reports he is not sure how long it has been going on but it has gotten “really bad” in the last 2 weeks. He states ” I feel like I am always going to the bathroom and have to get up at night every couple of hours”. He denies any blood in the urine or foul odor. He does not have any pain, fever, nausea, or difficulty starting the urine flow. He reports frequency, urgency. Nothing makes it better or worse and he has not done anything to treat his symptoms.
