Chief Complaint: Migraines History of Present Illness: Mr. Stovers returns to the clinic with a complaint of migraines along with worsening IBS, sciatica, and foot pain. No GERD. No chest pain or shortness of breath. No bowel or bladder changes. No visual changes. He is currently seeing a therapist for anxiety w/depression. Pt not suicidal at present. Had suicidal thoughts in the past. Migraines 9/10 in severity as described by patient. Occur weekly.
