A patient has Crushing chest pain & shortness of breath at rest that began 4 hours ago. The pain radiates to his left arm and is associated with nausea & diaphoresis. He is ashen in color. S4 gallop noted, decreased breath sounds at lung bases. ECG shows ST-segment elevation in the anterior leads. Labs show elevated cardiac troponin levels. Patient reports he has had intermittent angina over the past month but never sought medical attention until now. He has a history of peripheral vascular disease, hypertension for 5 years, and a 30-pack-year smoking history. He is taking rosuvastatin, amlodipine, and aspirin.
