This is a 60-year-old male that presented from a Nursing home after he fell today. He was found on the floor and complains of back, shoulder pain. Past Medical History. Significant for diabetes. Also has a history of CAD, and suicidal ideation. Labs normal. Urinalysis, negative as was C-spine, L-spine, and shoulder x-rays. VS are stable and he is afebrile. Alert, oriented, and appears in no distress. Lungs are clear, Abdomen is obese, soft, nondistended. Extremities show no bruising. Neurologically he is intact. Patient will be returned to Nursing Home and should be returned if symptoms worsen. Assign CPT code(s) for ED physician services.
