John, a 93 year old nursing home patient, had frequent chronic urinary tract infections and numerous courses of antibiotics. The most recent urine culture grew a Pseudomonas aeruginosa, resistant to all oral antibiotics. The patient was therefore admitted for IV antibiotic therapy. Significant in his history was the placement about 3 years earlier of a cardiac pacemaker for sick sinus syndrome. He also has comorbidities of emphysema, chronic respiratory failure, and diabetes mellitus with background retinopathy and neuropathy. The hospitalists determined that the oral diabetic medications were not controlling his diabetes adequately and added insulin to his daily regimen until stable. icd
