Case history: 61 y/o female found down at home by family. Found to have large left intraparenchymal hemorrhage. To OR for emergent L craniotomy and hematoma evacuation (intubated for procedure and a couple days after). Left bone flap out, will be replaced at a later date when the brain swelling goes down. As such, must wear a helmet when out of bed. CXR on Hospital Day 3 showed decreased lung volumes and areas of right basal atelectasis, possible consolidation. WBC count WNL for a few days, elevated on Hospital Day 4. On Hospital Day 4, consulted to initiate diet—currently NPO. What exam would you perform first? A clinical evaluation, an instrumental evaluation (i.e., FEES or MBS), or defer to another provider for a different type of assessment?
