Client is in rehabilitation after hip surgery. The client is alert, oriented to person, place, and time. The dient’s skin is warm and dry. Client reports needing to have a bowel movement but has been unable to do so for the last g days. The dient called the nurse to the room stating, “I have had an accident and wet the bed.
