CC: Heart burn. HPI: Mary is a 25-year-old white female who weighs 195lbs and is 64″ tall. NKDA. She smokes 1 PPD and is a casual drinker. She presents to your clinic with complaints of frequent heartburn. It started a couple of years ago and has been becoming progressively more frequent and severe. She is now having symptoms almost daily. She often has heartburn after eating. Thinks almost all foods cause it, but spicy foods are worse. If she lies down after eating it will precipitate the heartburn. She has chronic problems with constipation. She usually has a BM about once a week. Often has to take a laxative to have a BM. Has associated bloating and cramping. She has also noted a frequent dry cough. She has taken tums but does not get a lot of relief.
