SOLVED2482
Upon what consequence of cirrhosis will the health care provider base the decision to initiate drug therapy? Because portal hypertension (PH) is the first consequence of cirrhosis, it is also the first opportunity to initiate pharmacologic therapy. A large study identified that the initiation of drug therapy at the stage of mild PH (portal pressure <10 mm Hg) does not impact the development of varices in patients with cirrhosis. Based on this study, guidelines now suggest withholding treatment until gastroesophageal varices are identified. A platelet count >150,000 cells/mm3 in combination with a liver stiffness of <20 kPa based on transient elastography are the combined criteria to suggest a <5% risk of having high risk varices. A HPVG of >16 mmHg is associated with a higher risk of mortality.
