On post-op day 4 the patient begins developing shortness of breath, fatigue, and exercise intolerance. Repeat troponins are noted to be negative but chest x-ray shows pulmonary congestion and BNP level is 293. What is a possible explanation for this patient’s cardiovascular decline? Heart failure is one of the most common complications after an MI, especially in the setting of multivessel CAD. The patient’s decline is related to a surgical complication, and he may be having fluid overload. The patient is experiencing reinfarction. Pulmonary congestion is likely related to post-surgical atelectasis.
