SBAR for Ms A.R., 70 yrs. old female, admitted with elevated troponin, past medical history of OSA, RA, CHF, PVD. C/O cough, SOB, sore throat, chronic wound to buttocks, trunk, AKI, dermatitis under arms. Patient is on healthy heart diet, resident is A& X4, 2L/NC, tele baseline, BM since 2 days ago, has purewick. Resident has urine retention, bladder scan PRN >300ml urine then perform bladder scan, refused BIPAP, CT abdomen reviewed impacted stool. Refused OT/PT, Q2 turn, Q8 V/S. Urine sample requested for urine culture. Latecter ringer 100 ml/hr ordered, lab troponin 0.25. Patient weighing 345 pounds. A geriatric bed was recommended for the patient to promote skin integrity and comfort. An order was received for the patient to be transferred to a different unit with a geriatric bed in a single room.
