Client is a 76-year-old man brought into the emergency department (ED) by rescue personnel with his spouse at his side. His spouse found him on the floor in the bedroom at 0800. He was incontinent of urine, had slurred speech, and was unable to stand as a result of paralysis in his right leg. Client is alert but unable to determine orientation as a result of global aphasia. Client demonstrates appropriate strength in grip of left hand but is unable to grasp at all in the right hand. Noted facial drooping on right side of face. Spouse states he has a history of hypertension, diabetes, and atherosclerosis. Spouse forgot to bring in his list of medications but does recall he takes warfarin, “some type of statin,” metformin, and “two pills for his blood pressure.” Spouse states he did not take any of his medication this morning.
