CC: Migraines HPI: Sara, a 33 yo black female presents with complaint of migraine headaches. She has migraines for at least 15-years. She c/o mild to moderate headaches at least 3 times a week that are often present when she awakens and then last most of the day. For about 3 days, she has a headache that is a moderate to severe, pulsatile, located in the right frontal area, and is associated with nausea, vomiting and light sensitivity. This is similar to the headaches she has had for most of her life. She takes Excedrin 2-3 times a week for the severe headaches with minimal relief. Otherwise, she does not take any other medications. PMH: Migraines PSH: None Social History: Married, 2 children, works as a dental hygienist. Family History: Father-58- healthy. Mother- 56 yo. Migraines. Hypertension. Siblings- Sister- 31- migraines.
