Alzheimer’s: A 70-year-old female with a chief complaint of forgetfulness. Family notes a steady decline over the last year. Her husband died 9 months ago from lung CA. She has been living alone. Her family hx includes mother died from pancreatic CA, father died from MI, younger sister has HTN. Her routine meds include: lisinopril 20mg qd, levothyroxine 0.75mg qd, and fosamax 70mg weekly. She is allergic to PCN. She denies any complaints. Patient and family deny any recent illnesses or hospitalizations. Past surgeries include tonsillectomy as a child, cholecystectomy, and total abdominal hysterectomy. She is up to date on her immunizations except for shingles. She no hx smoking.
