He was deemed safe for a modified diet of minced and moist solids and mildly thick liquids. In the impressions of that exam, the SLP determined that he should perform an exercise to strengthen his swallow muscles (chin tuck against resistance) and some compensatory strategies to keep himself safe while eating the modified diet (two swallows per bite, alternating bites/sips). But he was discharged to the rehab before she could provide the swallowing therapy and education. The patient tells you his long-term goal is to get back to eating/drinking normally.
