Cellulitis/MRSA: A 54-year-old obese male presents with a 3-day history of painful red raised area on his right lower leg that has gotten worse over the last day. The area is warm to touch with mild swelling. There has been some tannish drainage from the area. He denies any injury or trauma to the area. He went boating over the weekend and was swimming in the lake. He only reports low-grade fever and mild myalgias. He has taken Tylenol without improvement. He does have a history of DMII, HTN, and hyperlipidemia. He takes metformin 1000 mg bid, lisinopril 20mg daily, and atorvastatin 20mg qhs. Allergy: Bactrim caused Stevens-Johnson syndrome. He smokes a couple of cigarettes per day. Drinks 3-4 beers on the weekends. He has had a right inguinal hernia repair at age 44. He is Methodist and attends church on the weekends. He went to Jamaica with his wife and 3 children in June. V/S: b/p 134/80, HR 86, RR 17, temp 99.1. Ht: 71″. Wt: 246lbs.
