SOLVED1445
Hypothesis/Nursing Diagnosis: Impaired Gas Exchange related to decreased oxygen saturation (O2 Sat: 85%) secondary to respiratory distress as evidenced by increased respiratory rate (Respirations: 26) and tachycardia (Pulse: 100). Short-Term Goal: Within the next hour, the patient will demonstrate improved oxygenation as evidenced by an increase in oxygen saturation to at least 92%. Nursing Action/Intervention: Administer oxygen therapy via nasal cannula at a rate of 2-4 liters per minute to improve oxygenation and increase oxygen saturation levels. Monitor the patient’s oxygen saturation continuously and adjust the oxygen flow rate as needed to maintain a saturation level of at least 92%. Additionally, assess the patient’s respiratory status, including respiratory rate and effort, breath sounds, and chest excursion, at regular intervals to monitor for any signs of worsening respiratory distress. Notify the healthcare provider immediately if there are any significant changes in the patient’s condition.
