Josh, a 12-year-old male, presents to the school health clinic with a complaint of a sore throat. He tells the school nurse that his throat was sore upon awakening this morning, but he figured it would get better as the day went on. He didn’t want to miss school today because he has a baseball game this afternoon that he really wants to play in, so he decided not to tell his father about the sore throat but now the pain is too bad to ignore. When asked, Josh rates the pain as a 7/10 and describes it as “sharp” and “burning.” The pain is worse when swallowing and he feels like his neck is sore too. He was trying to drink cold water during class to help relieve the pain, but it didn’t seem to help much. A review of Josh’s school medical record confirms he has no known medical or surgical history. He does have a documented allergy to Azithromycin which is known to cause rash. The school nurse notices that two other students from Josh’s class have been absent this week due to streptococcal pharyngitis
