Paul Windsor is a 4 year old who was admitted a few hours ago from the emergency department (ED ) with nephrotic syndrome . This is the first time he has had this disorder . His parents report that Paul had an upper respiratory infection about a week ago. They noticed that over the past several days he was lethargic and irritable with a poor appetite . He had some diarrhea and wasn’t voiding much so the parents were concerned he was dehydrated . However , when they got him dressed , they noticed that his pants and shoes seemed too small. His labs and urinalysis were consistent with nephrotic syndrome , so he was admitted . He appears to be appropriate developmentally but is ill-appearing . He has facial edema and significant pitting edema of the abdomen , genitals , and lower extremities . There are no signs of any areas of skin breakdown .
