Side Effect- An intended or predictable reaction of a medication. Examples of side effects can include dry mouth, headache, constipation, skin rash and dizziness just to name a few. A Few nursing interventions that may help with side effects could be to educate and explain how patient should exercise, drink lots of water, some medications you need to take with food, and taking a laxative. The Nurse should also check on patient every hour if not soon to see how they are doing taking a new medication. Adverse Reaction-When someone is harmed by a medicine. An unwanted or unpredictable reaction to a medication. This type of reaction can range from mild to severe to life threating. Some examples of an adverse reaction may include heart attack, anaphylaxis, hives, impaired hearing and or vision and kidney damage. Some nursing interventions for this type of reaction would be to stop the medication right away, give an antihistamine to help with itching or a rash, and administer epi to help with the anaphylaxis. You can also administer steroids to help with any inflammation.
