His B.P. is 160/100 and TPR is 101-86-32. He is dyspneic on slight exertion and is producing thick yellow foul smelling mucus when he coughs. A Foley catheter was inserted when he was unable to void and it has drained 800cc of cloudy dark urine in the last 24 hours. Directions There are more than 10 nursing diagnoses identifiable in the above scenario. Identify as many as possible. Be sure to include all three parts of the nursing diagnosis.
