CHIEF COMPLAINT: A 28-year-old single white male came in for treatment, saying, “I need serious help to stop using heroin/opiates, and it’s ruining my life: HISTORY: The patient returned from 9 months in rehab and halfway house program. Within 4 months of returning, she ran into an old acquaintance and ended up using again. She reports using four to five bags of heroin intravenously daily. His last use was 2 hours before the appointment. He reports being diagnosed with ADHD in fifth grade. He believes he was prescribed by his family practitioner, several different meds for this between fifth and eighth grades. He didn’t like taking them, said they made him feel ‘spacy.’ He reports smoking marijuana a couple of times in high school, but smoking and drinking was ‘never my thing.’ Around age 24 he was given ‘Oxy’ at a party, and ‘found my thing,’ using 15-20 30 mg tablets per day. When without the drug, he reports experiencing nausea, shakes, and chills. By the age 25, he entered a 28-day rehab program. He was clean for 8 months before relapsing with oxycodone again. Because Heroin was cheaper and easier to obtain, he switched. At first, he was snorting
