She states she has history of cocaine use and she checked herself in a Turk house and then was referred to All American program. She reports she is one month clean.She reports she is not on MAT program. She reports she been diagnosed with depression, bipolar, anxiety, PTSD, schizophrenia. She reports she was diagnosed at age 12 yo. She reports the last time she took psychotropic medication was two years ago. She cannot remember any of the medications that she was on.She denies any associated hospital admission. She reports past medical history of HTN, DM, Asthma, COPD, seizures, SUD, Cancer of the breast with metastasis to the stomach and appendix and she is on blood thinners. Of note she has been having pain and has been put on oxycodone and tylenol 3.Denies drug/food allergy. Her PCP is Dr. Jasmine at absolute care.She last show her a month.
