HPI: Sara, a 33 yo black female presents with complaint of migraine headaches. She has migraines for at least 15-years. She c/o mild to moderate headaches at least 3 times a week that are often present when she awakens and then last most of the day. For about 3 days, she has a headache that is a moderate to severe, pulsatile, located in the right frontal area, and is associated with nausea, vomiting and light sensitivity. This is similar to the headaches she has had for most of her life. She takes Excedrin 2-3 times a week for the severe headaches with minimal relief. Otherwise, she does not take any other medications. 1. Discuss the differential diagnosis of chronic headaches. Considering this lady’s presentation, what type of headache seems most likely? 2. Discuss the various treatment options you could consider for acute management. Explain the pharmacodynamics and pharmacokinetics of the various medication classes. What are some of the advantages and disadvantages of some of the medications used for abortive treatment? Discuss adverse effects, contraindications, and drug interactions.
