Chief complaint: Patient is a 74-year-old Caucasian female with a prior history of dementia and anxiety who was admitted due to severe psychotic delusions and visual hallucinations. Admission history: Patient currently lives in Highlands long Term Care facility since her dx of dementia. The Highlands Long Term Care gerontologist referred the patient to OT for behavioral training. It was reported that the patient had been in a state of extreme agitation and possibly experiencing hallucinations over a greater than 24-hour period. According to the patient, she was at the nursing home and had some conflict with one of the CNAs. She was unable to explain what the conflict was but admitted that she has been seeing things at the nursing home. She described how she saw things coming from the ceiling like a trough for feeding pigs and reported that she saw little black people who she felt were masturbating on the ceiling. She feels that it was her money that was being used to do those things that she saw and was upset about it. She has not been sleeping well at night. She also reports hearing ringing in her ears.
