1130: A full-term infant who was diagnosed with a VSD in utero during a sonogram. Birthweight was 3.12 kg (6.9 lb). Infant was previously hospitalized at 3 weeks of age due inability to feed and weight loss. During that admission, the infant was started on digoxin orally twice a day. Infant was seen in cardiology clinic two weeks ago and was doing well with digoxin prescription and parents reported infant was eating better. However, infant was still below the 5th percentile for weight on the growth chart at 3.65 kg (8 lb). Parents brought infant to clinic today because the infant was “breathing fast and was puffy looking around their eyes.” The infant is being admitted to the telemetry unit for treatment of congestive heart failure (CHF). Day of hospitalization: 1400 : Parent reports the infant is not wanting to feed and has vomited several times even though they have not had any formula since the feeding at 0500 this morning. They took 15 mL at the 0500 feeding. Infant is sleeping in crib. Day 2 of hospitalization: 1000 : General: Sleeping in crib. Easily awakened. Parent at bedside. Integumentary: Skin is dry and warm.
