The nurse reviews all recorded client data between 1715 and 1830. 1715 A 70-year-old male client presents to the emergency department (ED) complaining of pain, redness, and swelling to right lower leg. Client tells the nurse that he began to have pain in the right lower leg, which started yesterday evening. He reports noticing swelling and redness when he woke up this morning. He applied hydrocortisone cream, which did not help. Pain gradually worsened during the day, which led to his seeking medical treatment in the ED. Vital signs: Temperature: 102.1°F (38.9°C) Blood pressure (BP): 89/51 mmHg Right arm sitting, Heart rate (HR): 104 beats/min Respiratory rate (RR): 20 breaths/min pO2: 93% on room air Red, shiny area 8 cm x 5 cm to lateral right lower leg just distal to knee, which is very warm and tight to palpation.
