History of Current Illness: Patient presents with complaints of shortness of breath and bilateral knee pain of 8/10 upon exertion. Patient reports that she had previously been able to walk 45 minutes or more on her treadmill without any difficulty. About two months ago, the patient started getting short of breath while walking on her treadmill and now she is only able to walk 5 to 10 minutes before having to stop. Patient states her breathing improves when she stops walking or walks at a slower pace. She denies shortness of breath when lying or sleeping. Patient can perform all her activities of daily living independently but admits she moves at a slower pace than usual. The patient works full-time as a secretary without issues and cares for her ailing mother but admits to taking more breaks when helping her work around the house. Patient denies chest pain and palpitations. MEDICAL HISTORY: Patient admits to having been told her blood sugar “ran high” last year at a health screening but didn’t follow up because she didn’t “feel bad.” The patient states that she thinks her blood sugar was 288 at the screening.
