While working in the healthcare field, I have witnessed one medication error that was only caught during report by the transporting crew who was flying this patient to another facility. The patient was on a drip that was weight based and crucial for their diagnosis. The previous nurse and on-coming nurse overlooked the patient’s weight that was in the system and started this drip based on the weight in their chart which was heavier than the actual patient. This means that the patient was being dosed to high for the medication for hours. Once the error was caught, the provider was notified and the drip was dosed based on the patient’s actual weight. Because the patient was leaving the facility, we did not hear about an adverse outcomes. This could have been prevented by looking in the chart thoroughly and then assessing the patient before administering a weight based medication. The article and outcome for the patient in the article is unfortunate. Medication errors do happen which is why there are many verifications put in place before administering them. I do agree that it was negligent on the nurse’s part to overlook which medication was grabbed because how to prepare
