My clinical is in the community department of a CHHA-certified home health agency, Americare CSS. Together with my preceptor, Mr. Hu (a visiting nurse RN who has mentored many new nurses in practice). The target populations for the project are 55-year-old and older adults, mostly Asians, Hispanics, and Blacks. My research project is “Diabetes Education on Dietary and Physical Activity for Home Care Patients.” This study will increase the level of awareness among diabetes patients regarding healthy diet practices and physical activity, thus enhancing blood glucose control. My capstone project for the first three weeks is setting the topic, scope, roles, and timeline for the whole project. Based on the diabetes management study, I selected the diet and exercise recommendations since Carbone et al. (2019) found that they significantly impact blood glucose levels. My goals were to reach stakeholders out there, figure out what clinical issue needed addressing, set up a project plan, recruit a team of professionals to work on this project, and design an appropriate schedule for all the activities that should be done in the course of our cooperation.
